You can use the following sample letters to communicate with the different companies in order to try and resolve your situation:

To Credit Reporting Agencies; Inform Credit Reporting Agencies that you are a victim of Identity Theft.

To Creditor - I did not open this account or loan; Report an account or loan you did not open.

To Creditor - I did not make this charge; Report unauthorized charges.

To Bank - I did not make this debit; Report unauthorized debits.

Support Documents

Include a copy of these documents with your letter.

• Driver's License
• Police Report
• ID Theft Affidavit

Mailing Instructions

Send by Certified Mail, Return Receipt Requested.  This will give you proof that your letter was sent and received.
